Determined by a power analysis, 152 primary molars of 64 four- to eight-year-olds with at least two asymptomatic molars requiring vital pulpotomies were recruited, randomly assigned to receive CLOR, WMTA, or FC, and restored with prefabricated metal crowns. At the 12-month follow-up, recordings of 59 children with 142 treated teeth were taken for clinical and radiographic symptoms. Clinical evaluation was performed by a blinded calibrated evaluator, whereas four evaluators scored each radiograph for pathologies based upon a modified scale previously proposed. The data were statistically analyzed. At 12 months, 100 percent clinical success was observed with all groups. CLOR had the highest frequency of pathologic radiolucencies at 12 months. Conclusion. Aim: The aim of this systematic review was to analyse the existing literature on the effectiveness of herbal medicine compared to standard pulpotomy medicaments in primary molars. Only those articles which met the inclusion criteria were selected. Literature abstracts and full text articles were analysed in this review. Results: A total of six articles were included in this systematic review. All were randomized clinical trials which evaluated the clinical and radiographic success or outcomes of herbal medicine such as allium sativum, ankaferd blood stopper, elaegnus angustifolia, propolis compared with standard pulpotomy medicaments in primary teeth. The overall clinical and radiographic outcomes were not significantly different in all the studies. All the included studies recorded high risk of bias. There is lack of strong evidence to support the use of herbal medicine to replace standard pulpotomy medicaments. Conclusion: There seems to be a lacunae of strong and long term evidence to support the use of herbal medicines. An online survey tool was employed to send questionnaires to paediatric dentistry departments of 202 European dental schools. Email addresses were obtained from the European Academy of Paediatric Dentistry and questionnaires were sent to one member of each department in December 2014. The survey included questions on treatment choices and clinical scenarios where respondents indicated how they would advise students to proceed, given a particular case. Fifty-one responses from 22 different countries were obtained. The patients were followed up for 6 months for clinical and radiographic signs and symptoms to evaluate the success of treatment. Treated teeth were clinically and radiographically evaluated after 1, 6 and 12 months. A total of 75 healthy 3 to 10 yr old children each having at least one carious primary molar tooth were selected. Random assignment of the pulpotomy medicaments was done as follows: Group I, MTA; Group II, Biodentine; Group III, Propolis. All the pulpotomized teeth were evaluated at 3, 6, and 9 mon clinically and radiographically, based on the scoring criteria system. View Show abstract Assessment of the potential of Allium sativum oil as a new medicament for non-vital pulpotomy of primary teeth Article Full-text available Jul 2015 Shukry Gamal Mohammad Kusai Baroudi The objective of this study was to compare the clinical and radiographic effects of Allium sativum oil and formocresol in nonvital pulpotomy in primary teeth. Twenty children ranging in age from 4 to 8 years were included in the study. In every one of those children, pulpotomy was indicated for the primary molars. Pulpotomy procedure was performed and the radicular pulp tissue of one molar was capped with A. sativum oil in a cotton pellet while the other molar was capped with formocresol. The teeth were evaluated clinically and radiographically before and after 6 months using standard clinical and radiographic criteria. Statistically, these results revealed significant difference between the radiographic findings of nonvital pulpotomy in primary molars with the two medicaments. A. sativum oil has potent antibacterial properties that enable it to combat intracanal microbes in the infected pulp of primary molars. Better results were obtained when A. sativum oil was used. A. sativum oil had more powerful effects than formocresol on the infected pulp of primary nonvital molars. View Show abstract Global Burden of Untreated Caries Article Mar 2015 J DENT RES Nicholas J Kassebaum Eduardo Bernabe Manu Dahiya Wagner Marcenes We aimed to consolidate all epidemiologic data about untreated caries and subsequently generate internally consistent prevalence and incidence estimates for all countries, 20 age groups, and both sexes for 1990 and 2010. The systematic search of the literature yielded 18,311 unique citations. After screening titles and abstracts, we excluded 10,461 citations as clearly irrelevant to this systematic review, leaving 1,682 for full-text review. Furthermore, 1,373 publications were excluded following the validity assessment. Overall, 192 studies of 1,502,260 children aged 1 to 14 y in 74 countries and 186 studies of 3,265,546 individuals aged 5 y or older in 67 countries were included in separate metaregressions for untreated caries in deciduous and permanent teeth, respectively, using modeling resources from the Global Burden of Disease 2010 study. In 2010, untreated caries in permanent teeth was the most prevalent condition worldwide, affecting 2.4 billion people, and untreated caries in deciduous teeth was the 10th-most prevalent condition, affecting 621 million children worldwide. The global age-standardized prevalence and incidence of untreated caries remained static between 1990 and 2010. There is evidence that the burden of untreated caries is shifting from children to adults, with 3 peaks in prevalence at ages 6, 25, and 70 y. Also, there were considerable variations in prevalence and incidence between regions and countries. Policy makers need to be aware of a predictable increasing burden of untreated caries due to population growth and longevity and a significant decrease in the prevalence of total tooth loss throughout the world from 1990 to 2010.
